Upon completion, please fax the signed Agreement to (757) 265-9334 for processing and assignment to an expert. Upon receipt of the original Agreement, and the retainer check, one of our staff members will contact you for assignment details.

Complete the information about your case, and what you would like us to do to assist you in settling this case. You can either fax the completed form to (757) 265-9334, or email it back to us at com@forensic-analysis.com. One of our senior staff will contact you with the name of the engineer that has been assigned, and ask you for any special requests that you may have.
